[WASHINGTON, D.C.] – Assistant Senate Majority Leader Dick Durbin (D-IL) today announced that the Department of Health and Human Services (HHS) has awarded a $1,000,000 Chronic Disease Self-Management Program grant to the Illinois Department of Public Health through the American Recovery and Reinvestment Act.Today’s funding will be used to provide self-management programs to older adults with chronic diseases.
